Your Rights and Responsibilities

KSB Hospital shall inform each patient or, when appropriate, the patient’s representative, of the patient’s rights, in advance of furnishing or discontinuing care whenever possible.

These rights are as follows:

  1. You have the right to participate in the development and implementation of your plan of care.
  2. You have the right to make informed decisions regarding your care. This includes the right to be informed of your health status.
  3. You have the right to request treatment, and to the extent permitted by law, the right to refuse treatment.
  4. You have the right to formulate advance directives and to have hospital staff and practitioners who provide care in the hospital comply with these directives.
  5. You have the right to have a family member or representative of your choice and your own physician notified promptly of your admission to the hospital.
  6. You have the right to personal privacy and the right to receive care in a safe setting. This includes the right to be free from all forms of abuse or harassment.
  7. You have the right to confidential treatment of your clinical records in accordance with applicable law. You also have the right to access information contained in your clinical records within a reasonable time frame.
  8. You have the right during acute medical and surgical care to be free from restraints of any form that are not medically necessary or are used as a means of coercion, discipline, convenience or retaliation by staff. In cases of behavior management, you have the right to be free from seclusion and restraints of any form that are imposed as a means of coercion, discipline, convenience or retaliation by staff.
  9. You have the right to register concern regarding your care. The hospital has a process to address and resolve such concerns. For further information regarding this process, please contact our Director of Customer Relations at Extension 5524.
  10. You have the right to know the rules and regulations that you are expected to follow regarding your care and conduct as listed below.

Responsibilities:

  1. You have the responsibility to provide accurate and complete information about matters relating to your health and to report unexpected changes in your condition to the responsible physician.
  2. You have the responsibility to indicate that you clearly comprehend the course of action contemplated and what is expected of you.
  3. You have the responsibility to follow the treatment plan recommended by the primary physician responsible for your care, including instructions of nurses and Allied Health Professionals carrying out the physician’s orders.
  4. You have the responsibility to follow the applicable hospital rules and regulations, such as visiting, patient care, conduct and smoking policies.
  5. You are responsible for your actions if you refuse treatment or fail to follow instructions of the responsible physician.
  6. You have the responsibility to fulfill the financial obligations of your health care within the guidelines stipulated by the hospital.
  7. You have the responsibility to respect the rights of other patients, hospital personnel, the property of the hospital and the property of others.
  8. Understanding your own insurance policy and its limitations – to obtain a clear understanding of your benefits and any limitations of your insurance coverage, it is best to contact your insurance carrier directly for an explanation of the way your claim was processed and/or denied.
